The choice of acid suppression therapy should be individualized based on symptom severity, concomitant medications, patient preferences, and cost considerations. For otherwise healthy patients with intermittent heartburn, OTC antacids or H2 blockers may suffice. For frequent heartburn occurring more than twice weekly, an OTC PPI such as Protonix 20 mg is appropriate. For moderate to severe GERD, erosive esophagitis, or peptic ulcer disease, prescription Protonix 40 mg provides optimal acid suppression for mucosal healing. For critically ill patients or those unable to take oral medications, IV pantoprazole provides reliable acid suppression without the drug interaction concerns associated with other PPIs.
