The addition of fludrocortisone normalizes electrolyte balance, blood pressure, and renin activity in most patients with primary adrenal insufficiency. Quality of life measures are also improved with adequate mineralocorticoid replacement. Some alternative approaches to managing adrenal insufficiency have been explored, including continuous subcutaneous hydrocortisone infusion and modified-release formulations, but these technologies address the glucocorticoid component and do not eliminate the need for mineralocorticoid replacement. The development of new mineralocorticoid receptor agonists with more selective tissue distribution is an area of active research, but none have yet been approved for clinical use. Until such alternatives become available, fludrocortisone will remain the standard of care for mineralocorticoid replacement. The cost-effectiveness of fludrocortisone is excellent, particularly when compared to the costs associated with inadequate treatment, including hospitalization for adrenal crisis, emergency department visits, and lost productivity. Generic formulations provide additional cost savings without compromising quality. Patients should be aware that not all generic formulations are equally bioavailable, and they should try to obtain their medication from a consistent manufacturer to avoid fluctuations in therapeutic effect. Pharmacists can help patients identify the manufacturer of their generic medication and work with prescribers to specify a preferred brand if needed. Despite its age and the availability of newer medications for other conditions, fludrocortisone remains an essential component of the endocrine pharmacopeia.
